年輕人長期月經沒來－ 小心是否早發性卵巢萎縮 在最近門診的患者中，有部分患者是因月經沒來而求助於醫師，其中有不少比率是主訴月經沒來超過四個月以上，而這些患者的年紀也都不大（非更年期族群），到底是什麼原因導致其月經沒來呢？ 　　 月經沒來稱為無月經症，原因可分為原發性無月經症和次發性無月經症。原發性無月經症的定義是指年齡14歲以前沒有月經來潮亦無第二性徵的發育，或16歲以前沒有月經來潮但有第二性徵的發育者。而所謂的次發性無月經症則是指過去曾有月經，但超過三個月經週期或六個月以上月經沒有來潮者。其中導致次發性無月經症的原因很多，包括懷孕、高泌乳激素血症、甲狀腺機能低下或亢進、卵巢切除等，還有一種常被人忽視的是早發性卵巢萎縮(PREMATURE OVARIAN FAILURE)。 　　 在更年期發生的卵巢萎縮是一種自然的過程，但若發生在40歲以前則是屬於病理現象。早發性卵巢萎縮發生率在 35 歲以前約為 1/250，40 歲以前則約為 1 / 100。早發性卵巢萎縮並非指卵巢機能完全喪失，而是指卵巢機能不正常。因此這些患者可能會間歇性的分泌女性荷爾蒙、排卵；某些人甚至可以自然懷孕。另外早期診斷出早發性卵巢萎縮也有利於骨質疏鬆的預防，因為有近三分之二患有早發性卵巢萎縮的年輕女性其股骨頸的骨質密度低於同年齡的族群。因此對於早發性卵巢萎縮患者的評估與診斷需更加謹慎與積極。 　　 以下對於早發性卵巢萎縮的診斷與評估做一簡述與說明： ◎病史：雖然約有90％早發性卵巢萎縮的患者找不到其致病原因，但詳細的病史詢問及評估仍然很重要。包括： 缺少女性素的症狀：例臉部潮紅、陰道乾澀、性交痛、盜汗等。 卵巢是否曾開過刀？是否接受過化學或放射性治療？ 自體免疫疾病：甲狀腺機能低下、風濕性關節炎、第一型糖尿病、副甲狀腺機能低下、愛迪生疾病(Add ison disease)、重症肌無力等。 家族有否早發性卵巢萎縮的親屬。 家族中是否有心智障礙、發育遲緩的親屬。 ◎理學檢查：大多數早發性卵巢萎縮的患者在理學檢查中沒有異常，但仍需注意下列事項： 眼瞼下垂　　　　　 甲狀腺腫大 皮膚色素沉澱增多　 萎縮性陰道炎 特那氏症（特徵：身材短小、皮膚皺褶）。 ◎基本的血液生化檢測： 懷孕檢測　 泌乳激素(prolactin)　 濾泡刺激素(FSH)　 女性素(E2) 　　 一旦早發性卵巢萎縮被診斷後，其它一些檢測仍需加以評估，包括：抗腎上腺抗體（anti-adrenal antibody)、甲狀腺刺激素、甲狀腺素、細胞染色體檢查等。至於卵巢超音波以及卵巢切片檢查是否有助於診斷早發性卵巢萎縮，則仍有待探討。 　　 早發性卵巢萎縮該如何治療？基本上可分為兩大類： 給予荷爾蒙治療來防止因女性素不足所引起的不適症狀。 關於受孕問題的解決。 ◎荷爾蒙治療（女性素及黃體素)： 　　 若患者不想懷孕，可每天給予女性素及黃體素補充療法或每天女性素加上12天的黃體素補充療法。 　　 早發性卵巢萎縮患者自然受孕的機率很低，若想懷孕可先給予性腺刺激素刺激卵泡的發育，然後進行人工受孕或試管嬰兒等生殖技術的輔助。若患者本身卵泡沒有發育，則有賴捐卵來幫忙。 　　 此外，部分獲知可能是早發性卵巢萎縮的患者有些會有情緒及精神上的不安及焦燥，因此家人及朋友的支持和情緒上的安撫也非常重要。切記，早發性卵巢萎縮並非絕症，它還是可能有自發性的月經來潮、排卵甚至懷孕的機會。 　　 總而言之，若女性在40歲以前月經週期超過四個月以上沒來，血液中的濾泡刺激素上升，女性素減少，便有可能是早發性卵巢萎縮的患者，建議接受進一步的評估與治療。 每個人都有不同的體質，尤其是身為女生的妳，更需要了解自己的身體運作，給自己加倍悉心的呵護。透過生理痛和身體出現的各種小狀況，完全洞悉妳的體質特性。 型一：血液循環不良型 在生理期前會覺得肚子脹脹的，下腹部突出，一到生理期就會便秘。經血顏色為暗沉的紅色，感覺黏稠，有時會有像豬肝色般血塊流出。經血量多，第一天比較少， 但是第二與第三天起突然變多。生理期會達七天以上。因為血液循環不佳， 所以經痛時會痛到無法忍受，有時痛得無法起床走路，只能完全依賴止痛藥。如果每次都這樣的話，有罹患了子宮內膜症或是子宮肌肉腫瘤的可能。 ＊改善小秘方 血液循環不佳的人要多活動身體，要是工作累了，就伸展一下身體或是來回走動一下。要小心別受寒，不要吃冰冷食物，多吃溫性的食物。最好不要使用衛生綿條，這樣只會讓血液循環更差，使用衛生綿最好，而且要勤換衛生綿。最好多吃黑色、紅色、紫色的食物。生菜是冷的，蔬菜最好都是加熱處理。避免長時間坐著, 要多走路，讓骨盤的血液循環好些。可以喝些薑黃茶，或是中藥的玫瑰花、紅花、山楂子。 型二：怕冷型 一到生理期，腹部就有受寒的感覺，經痛嚴重, 一受寒會更嚴重，但是保暖會覺得舒服一點。生理期通常都遲來，經期常會持續七天以上，經血顏色是暗黑色的紅色，回夾雜像豬肝色般的血塊流出。特別怕冷，併發虛弱型症狀的機率很高。 ＊改善小秘方 要做好保暖功夫，尤其是下半身，可以穿厚內衣或厚襪。至於腰部以下，不要穿裙子，改穿長褲，因為要是下半身受涼，經痛會更嚴重。建議吃辣椒或紅蔥類溫性食物。生理期間若是覺得冷，可以試拋棄式暖包，分別敷在肚臍下方、 臀部分割線上方或腳底。平常可以盆浴或泡腳來驅寒氣。建議可喝薑母茶與肉桂茶，或是中藥的乾薑與肉桂。 型三：壓力過剩型 一到生理期前就會出現精神不安定現象，情緒變得很焦慮不安，容易發脾氣。貪食與厭食兩種現象不停地重複著，老是放屁或打嗝，會亂長痘痘，不是便祕就是拉肚子。每個月的經痛症狀不同，會隨當時的身體狀況改變，在經期前會腹脹或腹痛，但是月經一來這些症狀會消失。經血是一般的紅色，是正常的四到五天。有時會提早,有時遲來。 ＊改善小秘方 平常就要學習控制情緒，可以利用芳香療法來放鬆及運用呼吸法來鎮靜精神。日常生活作息要正常，生理期可聽聽音樂或喝喝藥草茶來安撫情緒。多吃橘子或喝茶，平常房裡可以放些綠色植物，佈置一個舒適的環境。起床後可以做些簡單的伸展操，如果有時間可以去散散步，在公園中伸大口呼吸。建議喝茉莉花茶與薄荷茶，中藥可試薄荷與陳皮。 型四：貧血型 容易頭暈，一站起來就眼冒金星，皮膚摸起來粗粗乾乾的，精神不集中，老是健忘。生理痛雖然沒有痛到受不了的情況，但會覺得腹部不舒服，還會腰酸背痛以及併發各種不適的症狀，而這這症狀可能會持續許久。經血顏色是粉紅色或淺紅色，經血像水一樣很稀，經期很短，約三到四天，或是以前是五到七天，但是期間越來越短。月經遲來的情況很嚴重，經常會拖到四十天以上。生理期快結束的那幾天會特別疲勞無力，即使生理期已經結束了, 還是覺得全身無力。 ＊改善小秘方 平常不要用眼或用腦過度，睡眠要充足，日常飲食生活要注意補血。每晚十二點以前睡覺，睡足八個小時，第二天也不要賴床。若是睡不著可以喝杯熱牛奶。可以吃動物肝臟或深色食物。工作一小時就休息十分鐘，到了傍晚反應會變遲鈍， 重要的事最好在三點以前做好，不要泡很熱的熱水或是泡太久。議喝棗子茶或枸杞茶，中藥可試當歸與龍眼。 型五：虛弱型 一到生理期前，腳就會浮腫，腫到連鞋子都穿不下，不是生理期卻出現不正常出血，容易疲勞，且腰酸背痛，不太有食慾，容易感冒或拉肚子。幾乎是不會有經痛現象，但是併發血液循環不良症的話，經痛就會非常嚴重。經血是淺紅色，有時量多有時量少，呈兩極化，有時會流出大量的粉紅色經血。經期通常是四到五天，周期會越來越短，若是併發貧血症，月經就容易遲來。這與貧血症一樣，經痛情形不嚴重，只是平常就覺得精力不夠，很容易疲倦，沒什麼體力。新陳代謝差， 水腫嚴重，尤其下半身更是肥胖。 ＊改善小秘方 三餐不能少，多攝取好消化、營養均衡的食物，像是五穀類與豆類。早餐一定要吃，因為腸胃比較不好，吃東西時要細嚼慢嚥。為了要有足夠的體力，一定要睡足八個小時，平常睡足八個小時，但是生理期最好多睡個一到二小時。虛弱型的人不適合做激烈運動，若是想運動，最好選晚餐後, 散步走路是最好的。建議喝杜仲茶與高麗蔘茶。中藥可試蓮子肉與黃耆。
